Making an Appointment
When you call our office to schedule your new patient appointment, please be sure to have your health plan member ID card available. If you have questions about whether we are contracted to accept your health plan we would advise you to contact the member services number listed on your ID card to confirm. As a new patient you may choose to print off and complete our new patient forms. Having these forms completed before your first appointment will help to expedite your visit.
What you can expect during your visit?
We ask that you arrive at least fifteen minutes early for your new patient appointment. At check-in you should be prepared to present your health insurance ID card and any forms that you have filled out from our website. If you have not filled out these forms in advance we will provide them when you arrive. Co-payments are collected as part of the check-in process. Please refer to our financial policy for information regarding our payment requirements.
Monday –Thursday 8:00 a.m. – 5:30 p.m Friday 8:00 a.m. – 5:00 p.m.
We do not close our office for the lunch hour, however, we are unavailable by phone from 12:30 to 1:30 daily. You can reach our answering service during this time and after hours by dialing 484-7386 and you will be connected to an on-call physician.
We see patients by appointment only. We strive to provide same or next day appointments with your doctor or with one of our physician assistants. Please call our office number to schedule your next appointment. If you need to cancel your appointment, we ask that you please give us twenty-four hours’ notice.
Our Office requires 48 hours notice for prescription refills. Prescription refill requests can be entered through our web site or by phone. Please have your prescription bottles available when requesting refills. We will need the name of the medication, the strength and the dosing instructions, as well as the name and phone number of your pharmacy. Patient Portal (link) or call (757) 484-7386.
We participate with most insurance plans. However, we recommend that you confirm with your specific plan that we are a participating provider. Co-payments are expected at the time of service unless arrangements are made prior to your appointment. We accept cash, check, Visa, or MasterCard.
We have a full service lab available in our office. Lab services for this draw site are provided by LabCorp of America. The lab is open Monday – Friday from 8:00 a.m. to 4:30 p.m. They are closed for lunch from 12:30 – 1:00. Labs are performed on a walk-in basis; no appointment needed.
